The Gujarat Occupational Safety, Health and Working Conditions Rules, 2025 have replaced the old Factories Rules framework — and with them comes Form 5, the Qualified Medical Officer's annual health examination report. We conduct the examinations and prepare the documentation for factories across Halol and the GIDC estate.
The OSH&WC framework changes what inspectors ask for. Under the Gujarat OSH & Working Conditions Rules, 2025 — which supersede the Gujarat Factories Rules, 1963 — the annual health examination of workers is documented in Form 5, the Qualified Medical Officer's annual health examination report. Factories that only hold old-format records will need to update their compliance files.
We structure every worker examination so it satisfies the new Form 5 requirement while still covering what legacy Form 32 / Form 33 documentation expected — because during the transition, auditors and insurers often ask for both. One examination, one visit, complete paperwork.
Each examination covers a general physical assessment, vision screening, audiometry and spirometry where the role involves noise, dust, fumes or chemicals, plus laboratory panels reported and reviewed by our consultant pathologists, Dr. Sunil Nagori (MBBS, DCP, PGCIH) and Dr. Shalin Nagori (MBBS, MD Path, PGCIH).
The Gujarat Occupational Safety, Health and Working Conditions Rules, 2025 are the state's rules under the OSH&WC Code, 2020. They supersede the Gujarat Factories Rules, 1963 for covered establishments and prescribe how worker health examinations must be conducted and documented, including the annual health examination report in Form 5.
Form 5 is the Qualified Medical Officer's Annual Health Examination Report under the Gujarat OSH&WC Rules, 2025. It records that each worker — especially those in hazardous processes — has undergone the prescribed annual medical examination, and it is the document inspectors will ask for under the new framework.
The 2025 rules supersede the Gujarat Factories Rules, 1963, under which Form 32 (certificate of fitness) and Form 33 (health register) were prescribed. During the transition, many inspectors and auditors still refer to the old forms, so we document examinations in a way that satisfies both the new Form 5 requirement and legacy Form 32/33 expectations.
Yes. We run shift-wise on-site camps inside your plant with collection material, audiometry and PFT equipment, or your workers can walk in to our lab at 2nd Floor, Dwarkesh Chambers, Near Bus Stand, Halol, Monday to Saturday, 8:00 AM to 8:00 PM. Worker walk-in batches run 8:00 AM to 4:00 PM.
Laboratory investigations — CBC, sugar, liver and kidney panels, urine routine — are reported and reviewed by our consultant pathologists, Dr. Sunil Nagori (MBBS, DCP, PGCIH) and Dr. Shalin Nagori (MBBS, MD Path, PGCIH).
